When players log in to play ESO they get a system message.
"Thank you for playing ESO. We have decided rather than taking our direction from an extremely vocal minority who comes to the game boards to rant, we will be giving weekly surveys. If you do this brief survey, you will get a (pet) which has the duration of one week."
Your options are a) opt out or b) do survey and get pet.
The next week, when the players log in they get a system message.

Your options are a) opt out and feel shame for killing off your pet or b) do survey and get one more week of life on to your pet.
The technology for this is extremely simple.
I believe the only reason game companies don't do this is a) apathy and b) laziness.
Heck, if I were the company I would do it even if the results went into the garbage can because then I can say things like
"We'd like to address your issue, but our weekly surveys show that the majority of our customers feel we should be working on this other issue now, but rest assured - it is on the list."
"We care what our customers think - that's why we give them a weekly survey!"
