Severe Input Lag Spikes

Turning on the FPS shows it is maintaining a average of 30.. however often in large groups i am seeing my camera stop tracking the mouse for protracted periods of time..
In addition, it seems the server side lag is extremely bad, with combat often seeing large jumps.
My Internet is fine, My frame-rate is fine... My CPU is spiking to only 75% at most... (its the oldest component in my computer...)
so WTH is causing this lag with input?
