Dual Wield, Poison Arrow Issues

I've seen this in other posts but can't find the topics on it. 
Since the last patch I am having a horrible time using dual wield. I can hit it one time and it works but after that I cannot swing my swords at all. I have to alt tab out and go back into the game to get movement back. This problem was very random since the beginning but it has become a constant since the last patch. I've tried different characters in different areas of the game and it's constant.
I've also noticed using #3 to shoot poison arrows like at an anchor drop, it works the first time then after that I cannot shoot. I have to switch weapons and go back to my bow to get it to work a few times then it quits again. I can shoot arrows normally without issue and I can use my swords normally without issue. It's just when I try to dual or use poison arrows that I run into problems. I have verified many times that I have full stamina and magic so that isn't the reason they are not working. Also I do not use any addons for the game.
I am using a Logitech G510 Keyboard and using the game default slots for dual wield, arrows etc. This has only become a constant issue since the last patch. Anyone else having this constantly?
