both of you leave the dungeon (if your in a dungeon) then both go back in OR finish the quest that has you in different instances OR play it like a single player.

    NO.
You're not in phase. when you're grouped with people...you need to tp to them so you're in phase. click p....right click on the person and click "travel to player".

    Problem solved

If it persists. Leave group, log out, log in and repeat the above.

    not entirely.

in many grouping cases it's due to not being on the same section of a chain quest. ie. on many occasions my group member will disappear after they complete a sub-portion of the quest we're running. As soon as I also complete that section and we get in-sync we suddenly become visible to one another (no travelling to player required.)

it appears (no pun intended) to be purposeful in terms of design rather than be a phasing error.

I believe the OP already indicated they had tried that.

You are likely on a quest, such as slaying Dothia for the Fighter's guild that disallows friends to aid you in the dungeon.
    It's a feature.

A lot of the open world quest chains rely heavily on phasing, an area may change depending on how far along in a chain you are such as NPCs becoming hostile/friendly or disappearing altogether. So unless you are on exactly the same stage of a questline, your group member will appear as an arrow even if they are stood only a foot away.

This is fine if you don't mind questing alone (I actually enjoy the living world feel based on the decisions I have made in questlines) but it is fairly counter-intuitive to questing with friends.

Myself and a friend began questing together in the early days to benefit from the Rings of Mara, but we soon got out of synch with differing playtimes and gave up in the end because it became more hassle than it was worth trying to decipher where we were in quests and which ones we could complete together.

it is also possible to be on the same step... BUT have chose different paths /endings for a quest (one of you picked poison the peasants, the other picked save them). sometimes this creates 2-3 quest steps near the end of a quest where you will be out of phase until you both finish, other times it affects an entire town (your town is all zombies forever, his is not), and you can link up again by crossing a border outside town.

It can be really tricky to stay in phase with others with some quest areas depending on how the quest was made and how it branches /ends.

Like others have mentioned in the past:

Most of the time, you are doing your own adventure while people around you are doing their own adventures.
Grouping up is more for things like dungeons and stuff.

Now, I must say that there must be something weird about this here. My partner and I have been doing some grouped Glenumbran questing over the past couple of days, so that after every quest that we finished we should be in sync with each other. Each day we end in Daggerfall to reap our spoils, get better gear, better weapons, etc.

Then the next day we log on to the server separately and each of those times, neither of us show up as our character to the other, but always as just an arrow. Then one of us has to relog to get that fixed. Usually the person that logged on second. It also happens that we're ungrouped, that we have to regroup first, and that after we're grouped we're invisible to each other. So there is something fishy going on there.
