Getting short lags when doing quests/ open map/ talking/..

Good Evening,
I recently got a problem:
Everytime something changes at a Quest ( Accepting, progressing, entering quest area), i get a lag for like half a second or a second.
This also happens sometimes when:
- I open the map
- I open the inventory
- I talk to NPCs
- I'm looting quest related stuff
I monitored my hardware and i could not see any spikes at those moments, my CPU/GPU usage is fine.
The problem started out of a sudden yesterday, I only remember applying a new GPU driver, but I already uninstalled, reinstalled, reinstalled the old one etc. but it did not work.
I also did everything mentioned in the "common issues" post and also changed every visual setting but they are not responsible.
My question now is: Does anyone else have this problem? Does anyone know a solution?
I maybe have to mention that I changed the
SET RequestedNumThreads "-1" to 0 because I got those 5 second lagspikes earlier, but they were CPU related and the new problem isnt, also, everything worked fine until yesterday.
I hope you can help me in this case, it's really annoying if it's happening so often.
EDIT:
I run the game fine at 50-60 fps so it shouldnt be a graphics problem

✭✭✭Same here, with/without addons. It gets worse when an addon uses map pins such as ZrMM, but it happens a bit regardless. The fewer quests I have in my journal, the better it gets in terms of how long/bad the hitch/stutter/pause is. I have already tried a repair of course.
