Sometimes you can have partial internet loss.
Open a command prompt and type.
ping -t google.com
It will give you a continuous ping letting you know if you are getting lost packets. If you are getting lost packets to google then it's your internet.
Not saying it's not them, but that test will let you know that everything is fine on your end of the internet.

this returns 90-100ms consistently, so I believe that confirms everything's fine on my side
Out of curiosity I connected my laptop to the internet through a wifi repeater device (http://www.amazon.com/EasyAcc®-Wireless-External-Portable-Smartphone/dp/B00CSFCYFY/ref=cm_cr_pr_product_top) to see if the lag persisted. Lo and behold, the lag cleared up entirely. I'm not sure if that just means something's wrong with my router settings or what, but I believe it may be a crucial hint. Anyone have any ideas?

I've heard of old routers and routers with bad programming causing problems like this, which only affects certain programs. Is your modem old? Have you looked for firmware updates?
Another thing to check in to ensuring ports needed by this game are being forwarded. You could briefly enable DMZ mode in your router, if it has the option, to see if it goes away. That might indicate it's some weird port issue. Although in general ports either let something through or they don't. If they don't it shouldn't work but in troubleshooting, being thorough is a good idea. If it did somehow work, it should not be used a fix as it essentially tells your router to let everything through. Like a bank without a vault where the money is just piled up in the middle of the floor.
