Grouping is not working

In my current group...
1. Group finder throws four dps into a regular dungeon - role selector seems to do nothing.
2. Instead of everyone entering one dungeon instance we end up in three separate ones and need to teleport.
3. The icons above everyone's heads appear in random places and several are huge making it impossible to see in front of me.
4. The leave group option doesn't do anything.
These issues were all being reported late last year in beta (except for the huge icon bug maybe).
