✭✭✭✭✭Both Solo Dungeons have a name swap in their conglomerate achievements.
They are both looking for the "Master" achievement, which does not exist. Both do have something named "Masochist" though, which I assume is what is intended.

✭✭✭✭✭Another thing - not achievements, but related to achievements.
This has been irritating me for ages: the Ritual Circle Totem memento from getting the Kyne's Aegis trifecta is a rare enough achievement and a great thing to show off, but two of the totems are swapped.
The "Fire" totem has a gargoyle head and the "Rock" totem has a dragon head. In every other place in the game, Dragon-headed totems breathe fire and the gargoyle-headed totems throw rocks. While the memento has the effects right, it swapped the models for those two.0
