Stuck doing ice heavy attacks

While I'm trying to weave lights, my character will sometimes start a heavy attack that I can't cancel, even if I let go of the mouse button, try to cast a spell, or try to block. I'm using an oakensoul ring, so it has nothing to do with weapon swapping, and it only happens during combat when I'm trying to weave, so I am pretty sure it's not my mouse. Is there some setting I accidentally toggled or something? I can't even replicate it on a dummy - it only happens when I'm actually fighting things, especially in a dungeon.

This happens sometimes… I once accidentally pulled a boss early because my character wouldn’t stop heavy attacking. I don’t think it’s anything you’re doing. Do you have high ping? Maybe thats affecting it?

No, my ping is only around 100. It's making tanking a pain when I can't block or cast because my character is stubbornly doing a heavy attack.

I've noticed this happens to me when the mob that is being target by the attack moves from in front of me, my character will get locked into the heavy attack animation and do a kind of slow turn towards the target until it goes off.
