Guards - they seem to move in font of you as you approach.
There's an NPC in Auridon that just stands right in the doorway of some outbuilding with a chest in it. You can't get out.
Then there's the NPCs that walk through you whilst teleporting; I often do something quick while the port is happening and come back to find that I'm still there...
