I've been having these issue with my main (HA sorc) for a long time now. If I'm not at the right angle, my sorc just stands there doing nothing when I hold the left mouse button.
Xbox EU.
I used to hold HA with lightning staff till an enemy died and then keeping it hold down till my aim goes to the next enemy and it automatically fired another HA.
Not working like this anymore, at least not always, often fails and I have to let go HA and press it again when I have the next enemy perfectly in my aim.
I agree it is frustrating and want it back, but I can see people would claim it was not working properly before.
For sure this new change negatively affects combat performances, especially in Infinite Archive and hard content. Hope it gets reverted...
Yeah, the heavy attacks have been busted for oh, I dunno... a couple of years now? I and many other reported it both on the PTS (many iterations of the PTS in fact) as well as on live. Its not getting fixed. Put it in the bin with the rest of the "won't fix" issues. Whatever they changed that causes the inputs to drop was more than likely intentional and they have absolutely no desire to undo it.

Adjusting or simply turning off "Arcanist Aim Assist" might be worth a try (Accessibility menu).
I had a few weird aiming hiccups with fiery grip and other ranged skills after Necrom dropped and setting the mentioned option to zero resolved it.
