I have done the log out and back in thing multiple times and didn't fix the situation for me.As already said, it's not a quest bug, it's a server respawn / interactions issues... Quest are working fine at the opening of the server then things are going bad quickly as the time go through...
I think they're well aware of this, but note that for 80% of these bugs, loging out then back in a few times generally make you reach a phase which is not bugged... Hope they finally find a way to solve this because it's really killing the game experience.
As for the crash when entering new areas, I didn't had any crash since 2 betas nor since the release...

If it walks like a duck and quacks like a duck ... That it's one giant bug in the basic technology actually doesn't help. I remember on the other forum how people who didn't take optimistic 'all fixed' patch note noises as gospel and suggested another beta test just to check were howled down by a mob.
It's a great game and I'm subbing but launching with an untested beta is not the way to go for good reviews and word of mouth.
But I guess paid for 'Early Access' is how the cool kids are pronouncing 'beta' these days.
