video graphics issue: vsync and stuttering issues

so this all started back in the beta where i noticed that my skype messages were popping up over my game.
something i found rather odd.
i also noticed that if i use vsync on nvidia control panel/ingame vsync/d3doverider i get terrible screen tearing.
if i turn it all off the windows aero vsync sets in.
but the problem with that is that im pretty sure its giving half refresh rate.
i also remember that back in the day there were a few games that tried this method.
with the not true fullscreen thing.
it was a disaster.
any solutions would be great, so far the game is very good and fun.
