Well how does it happen with NPCs?

Do they bite you like players or just random NPC attacks and you realize that you get infected?

went around till I spotted them, approached them, and let them attack me, I got a message that poped up on the left side of my screen telling me I was infected, and to go to the nearest wayshrine/

worst part was finding the quest after getting infected, I'm kind of stoked, asked about it, and got infected in the course of a few hours, I wish this same RNG luck applied to when I'm farming gear and weapons!

You get bit / scratched by bloodfiends out in the wild and get infected with Noxophilic Sanguivoria. You then pick up a quest that allows you to find a scroll, which brings you into the quest.

However, they only spawn sometimes and in some places, so if a player is looking to get infected "authentically", it requires more patience than simply asking for a bite.

I only ever get "natural" bites ('cause I want to be the first in my own bloodline) - so, I have found WW & Bloodfiends in all 3 zones, and have made marks on a screen-shot of each zone's map to remember the locations - but I find Bangkorai to be the easiest / most reliable. I only ever port to Troll's Toothpick wayshrine, and then follow the road down until you hit the crossroads that lead to Evermore. There are 4 spawn locations visible from the main road on this route. Then I use East Evermore wayshrine to port to Old Tower wayshrine - and head east from there until you hit that enemy encampment that kind of guards the road that leads into Craglorn. There are 3 spawn locations on this route. If no one has killed the spawns, they sometimes remain even after it is light out. I usually ask if there are any natural spawns spotted in zone chat as soon as I get there - and while usually no one answers - I have had a few people whisper me their locations while they waited for friends to come and get natural bites. Good luck!

Edit: as stated above - if you do find them on your own, or are shown their locations - do the right thing and just run away after they have infected you. Do not kill them, please.

I'll keep this in mind, if I decide to get another bite, I did get bit in iBangkorai a few hours after making my initial post near trolls toothpick area , the hardest part for me was figuring out where to go after, game says go to nearest wayshrine that didnt help at all, was nothing at the nearest wayshrine

It's a rare occurrence but it's guaranteed to happen. There was once a pet on the crown store that had it's eyes glow red on these special nights, but alas it's no longer on sale.

This used to be the case. During the 2020 Halloween event, the goat was broken (I don't know if this was intentional or not), and the eyes glowed throughout the event. Since then, I haven't seen them glow at all, though I have been using the goat less in recent months.

Is it a wayshrine or the Vampire shrine? If it's the Vampire Shrine, it's in a nook in the rocks near the crossroads NE of Pellin's Graveyeard (west side of the road)

Yeah, it's worth knowing that the infection spawns don't all pop up simultaneously. A few of the nodes go active. So, you need to run a circuit checking multiple spawn locations (ideally as quickly as you can.) In the past (back in 2014) it seemed like they would respawn a couple times throughout the night at different locations, if they'd been wiped out. I'm not sure if that's still the case, and the understanding at the time was that they didn't respawn at all, even though this was not completely true. They simply wouldn't respawn at the same location.
