So, is a workaround for this to make sure that companions are not summoned when going to or from a dungeon or trial instance?

Make sure to unsummon them manually before joining the dungeon finder or dungeon/trial group.

Similar issue this morning. I had Mirri active when I ported into a random dungeon (Castle Thorn). I had not checked her gear before this, but I did last night before I logged off. The tank left immediately, and Mirri became active again. During the first fight, I noticed she was fighting with her fists (no weapon). The group broke up after a few wipes, and when we arrived back in Mournhold I checked and ALL of her gear was gone. I logged to another character and it was still missing. Logged out entirely and back in - Mirri's gear is still gone. Activated Bastion (I've unlocked him but done nothing with him since - Mirri is best girl!) and he still has all of his starter gear.

Do I need to submit a ticket for this, or is posting here sufficient?

If you are hit by a bug... always submit a ticket, even if it is reported in here. Tickets get into their system directly and immediately, and each one adds weight to the issue. Threads in here are pre-chewed by the Community Managers and delivered in summary. Both end up being reported to the development team, but I personally think that the reported bug or ticket is more valuable.

I went into the dye station and was fiddling with costumes and such to get the companion achievement for customizing Mirri's outfit and all her stuff disappeared .. her weapon (box) and all her armor .. and lost a shield that wasn't equipped. Lost 3 items I had gotten better versions of from in-game drops. Had to go re-buy ALL her equipment. (PC/NA)

I haven't lost any gear yet but i noticed that the companions are always naked for a split second every time you travel.

I'm guessing ESO makes a call to the server about their gear every time you zone into a new area and if that call is lost in space, your companion stays naked.

That is some next level bad game design right there ...

If it were just "lost in the mail" then the next time you zoned, the items would return.

Happened to me I got her some lovely blue rings & a weapon etc & it all disappeared. Think it was when the game decided to port her into a dungeon when someone left she went naked for a second then later her gear was gone.

Tried to lock her gear after that but it doesn't let us

Lost two blue two green gear on MIrri when I queued for a random normal with her active and then had to log before I got into the dungeon. When I got back in game, all her gear was gone.

Funny as hell watching her fight mobs with her fists!

Prehaps disable companions in daily random dungeon que, even the random dungeons might help narrow down this issue, and prevent future players from losing their stuff, as there probably a large number of the player base that isn't aware of this issue.

This is the forth time this bug has happened to me in the last 2 days.

I'm trying to help so please note the following

I have NOT been in any random dailies or any quest other then the quest that unlocks the companions (on my level 23 character). ..
I have unlocked the companions on other characters that are around 800CP
I was on my 800CP with the companions out when I logged out to switch characters (I think this might be part of the error)
logged into my level 23 character and did the quest to unlock the companion
After the quest was done and I left the quest area, I called the the new instance of the companion... lookie lookie he is showing off his tattoos
This leads me to conclude it is NOT just quest or dailies causing the problem

Something that might help is make sure you UN-Summon the companions before logging off or doing anything... being there might be subroutine that gets called to save the state of the characters companions. I believe this is a logic error, the game sees that you have the companions but does not know until you unsummon the companions what they have on or had on... basically the character is saved BUT the items are NOT

Best of Luck to everyone and to the team trying there best to help and pinpoint the errors and fix them... thank you

From the patch? No(Highly unlikely, since the items seem to be completely gone and not f.ex. invisible).

Mirri lost all her gear. Last night.

Joined a random normal daily, finished, zoned out, Mirri reappeared with no gear.

Because she had a costume on, I did not notice until I realized she was punching in combat.

The fix will not have restored your gear. It was only to stop more gear from being lost.

If you want to pursue getting that back, you will need to open a ticket with support — or reply to the current one you have. It may take a few back-and-forths, but be clear you are asking about the gear itself

I am guessing that requests to restore the gear will not be honored. That gear is gone. Mirri left it back in the room and the maid stole it.

What I am interested in is whether new cases of losing gear are fixed. They cannot join me on my adventures until I am reasonably certain that this hotfix actually fixed the problem.

I’m in the same boat. So far the only report I’ve seen about companion gear since the fix is a problem where it can’t be retrieved from containers:
