Constant Freezing on PS4

Why am I constantly freezing having to close down game to restart and then taking 5 min to log in because it says I'm already logged in I'm literally freezing in empty servers with no one around no less then 10 times a night I have fiber internet that never go's below 300d100u speeds and my Nat is open this is really getting to the point where I'm about to say screw it and move on
