How does one appeal or escalate forum moderator decisions?

I'm getting discipline handed out to me, by the same moderator, with associated comments that I'm failing to reconcile to objective reality.

What are my options, if any, for requesting redress?

There really aren’t options for disputing moderator decisions. In fact, discussing moderation is against the code of conduct, and this thread will be closed simply because you asked the question. Sorry.
