They should not attack friendly or neutral npc unless they turned hostile. The problem is that even though I have prevent attacking innocents on the shade will still attack npc's from time to time.
If the shade attacked npc's when you had prevent attacking innocents off like you explained, I do not think that could be considered a bug.

Not going to lie some separate setting for these pesky npcs would be amazing.
