It is primarily the same with two big changes:
1. It revamps the "Kill Worm Cultist" part by granting you every new Vampire Ability and making you use them on the Worm Cultist, one after the other. (With additional dialogue and explanations inbetween)
2. Lamae Bal has new dialogue, wherein she states that her Blood Scions are magically engineered by her, so that they grow more powerful by feeding rather than starvation. She also further provides an explanation on how to "live" as a vampire.
The biggest thing here to me, is that they do explain by lore now, that most vampires grow stronger through starvation, rather than feeding. This displeased Lamae, so she changed this for her Blood Scions through extensive magic and rituals - apparently. It's an excuse, but at least one doesn't ruin past incarnations of Vampirism.

Good, Vampire Lord was never stated to be exclusive to the Volkihar Bloodline, Harkon stated it was a purity thing.
What is creative about not having wings yet still looking practically the same as a vampire lord?

Nothing in ESO can fly yet except npcs. I would agree with you if Zos made flying in eso a form of traveling (flying on brooms, dragons, having wings on yourself, ect.), but since they didnt, of course they are not going to fly.
