I'm left-handed and i use the mouse (Left Handed Naga) with my left hand and use my right hand for the keyboard.
My key-binds are:
Forward: (Physical) Left Mouse button (mapped to logical right button because left-handed mouse)
Attack: (Physical) Right Mouse button (mapped to logical left button because left-handed mouse)
Back: Down Arrow
Strafe Left: Left Arrow
Strafe Right: Right Arrow
Jump: Up Arrow
Sprint: Num-0
Crouch: CTRL
Interrupt: Right Shift
Skills 1-5: Backslash | Delete | End | Page Down | Num-4
Ultimate: Home | Key on Naga Keypad

I cant even fathom playing this game without it. I use the 12 buttons for all my skill slots, roll, break free, and several quickslots. I use my left hand for movement, and my left thumb does bar swapping, tab targeting, and jumping.

I do have a gaming mouse (Logitech G502 Proteus Core), but I don't have any of the buttons mapped for this game.
I do make use of my Logitech G11 keyboard, though.
This set-up keeps most of the most frequently-used buttons within very easy reach of my fingers, without moving my hand.
I also have some of my add-ons hot-keyed.
There is one shortfall; I don't have the 'D' key mapped to the G11 number pad, so I have to make do with moving forward, back, and left. I have the Spacebar mapped (for jumping) to where the 'D' key would logically be, in relation to where I have the other keys set.
I've gotten so used to having the keys for my games mapped to this keyboard, I'm gonna be hurtin' when it dies, if I can't find a similar keyboard to take its place.

Attacks and Skills are on buttons 1-6.
Buttons 7-12 are harder for me to reach (I have tiny hands, even for a girl) so I use 7-12 for macros. (Mostly crafting macros, and posting things like "Chest" to group chat in dungeons. Unfortunately combat macros don't really work - miss one weapon swap due to lag, and every skill that fires after that will be totally wrong...)
Light/Heavy Attacks on the left mouse button, Block in the middle. Makes attack weaving and animation cancelling super easy. You do it all with just your thumb and index finger.
Right mouse button - click to Interact with something, hold down to Move Forward. Just so I can run around and gather things with one hand, and drink my coffee with the other.
Keyboard hand - WASD, Weapon Swap, Sprint, Crouch, Interrupt, Block. Basically anything movement related is the keyboard hand, and anything attack related is the mouse hand.
Warning: I do recommend this mouse. It will change your gaming life. But once you get used to it, it will be impossible to ever go back...

At a minimum, I can start by using the extra top button and the middle button to separate the weapon switch to two buttons. That way, if I want to be extra sure it switched to front bar or back bar, I can mash the button. With a single weapon swap button, if it doesnt switch because of lag, then you mash it, you never know what you're going to get...
