Ultimate not generating

So my friend just started a stamDK and was having a blast for a while, but for the last couple of days she can't seem to gain any ultimate. We are usually grouping together but she does plenty of damage. We've tried relogging, changing weapons, repairing, charging, etc. Tried using different ults. Nothing.

Anyone else have this problem before? If so, how did you solve it?

The simplest way to generate ultimate is just light or heavy attacking. When you damage an enemy with a light or heavy attack, you will receive a buff that grants three Ultimate per second for eight seconds. This buff is refreshed with each light or heavy attack. If this buff is not activating for your friend, please have them remove addons in case one is affecting combat.
