Jump instance failed

I’m be been queuing for white gold tower veteran on Xbox and every time it finds a match for me after it loads the map it says jump instance failed and then I get kicked from the group a little after. I’ve restarted my game twice and it keeps doing this for hours. What gives?

I'm on PS4 and honestly this happens so frequently it is amazing they haven't actually sorted it out.
Normally we can just travel to player or jump in from a wayshrine directly into a dungeon but sometimes we cajn't do either of these and we have to quit group and face a 15 mins ban.
I have never triad actually travelling to the dungeon and entering that way though.0
