There are already some NPCs who can be seen using the bathroom going out in the open.
If you go near Shark's Teeth Grotto in Hew's Bane, there is one particular enemy NPC who will go up to a wall, stand against it for some time, do a little shake, and walk back. I thought it was a rather nice touch

I always wondered why fences are so interested in paying premium prices for the used chamber pots we bring them... (Medieval times.. in ancient times like rome or Greece they did have proper toilets)
Perhaps you should learn some history before talking about something... because back in medieval times, they had chamber pots and would simply be emptied out of the window onto the streets below. Yes, the smell was most likely horrendous and was a reason why diseases ran rampant in cities. Keep in mind people also rarely bathed, perhaps once a month at most.
