While there may be a bug in the phoenix set, there is another possibility.
A player's health bar is calculated locally in the game client, and the status of such is sent to the server, which then propagates said status to other peoples' game clients. In the time that this is performed, your own client performs the same calculation, and if a hit SHOULD kill someone then it shows them as zero health on your client, though they are only dead when their own game client tells the server that they are dead (and then you see them as really dead). One of the ways this works is that the game code is not obfuscated well in memory, so it's possible to read/write to the health bar buffer such that a near continuous calculation is performed on incoming damage: If %incomingdamage is greater than %currenthealth then set %incomingdamage to %currenthealth-1
This keeps the person at 1 health (thus not dead) minimum. The way to kill them is to get enough simultaneous damage on them so that the process that would save them (otherwise keeping them at 1 health) misses one of the damage calculations. This is the same way that certain players (who we all know) kept their resource pools forever full in the last patch.

I killed a Templar wearing this set (twice) just fine. He stayed dead the second time.
There is obviously something being done to these players causing this bug. We just have to narrow it down.

If the person is knocked downed 100% it will bug. I'm not sure if this applies to stuns too.

Every time i killed her (or didn't), her HP would stop at 0% and she would not die. 4 times in same fight her health stopped at 0% even though the damage of the hit would have taken it deep into the minus.
