Repeating quest bugs?

I really hope I posted this in the right place 
I have an issue with certain quests repeating after being finished, but no way to do them...really, cannot even attack or be attacked
The most recent just happened before I posted this.
It was a quest to take a scout to a ritual site and stop the wormcult from completing it.
I finished the quest, got paid...but there was a portal by the woman that charged me with the quest, so I entered it to find myself in a home in Riften.
I went back through the portal and she told me to repeat the quest, only this time the guide was female and the ritual was different.
When I reached the site there were cultests everywhere, but none attacked me and I was not able to attack them...like I wasn't even there.
On the ritual site was a big stone table with what looked like a Glenhaven witches head on it (the witches from Skyrim)
Anyone else having these issues?
