Double + mundus stone bug

It seems to not have been fixed. I know people who are still able to equip double mundus stones without having the Twice Born Star set on them.
I know on live someone who got 4 different mundus stones on. When he would get a new one, the previous one would not erase itself and ended up with four. He opened a ticket and asked for them to be fixed and removed before he is branded as an exploiter so ZoS should have the info on how he did it.
I'm unaware if its still going on in PTS though. But the double mundus one is still happening on PTS.
Would be nice to have an official update on that matter.
