Its how you use it.
In a more realistic setting, that 'hole' in the khajiit shields would be used for a spear, so the shield can be held in front of you and used for blocking at the same time that you're thrusting with the spear at your opponent. Notches or holes were often put into shields used in spear, pike, and lance formations for that reason.
Problem is the animations in this game are in no way even close to actual combat techniques, and they left out spears and polearms =/

higher level shields tend to be bigger.
also, different racial style shields come in different sizes.
