Bat Swarm ult sometimes not activating

-Contains a story spoiler - I stopped playing ESO after the first month and only came back two days ago, so I might have missed an update. But currently, sometimes when I try to activate Bat Swarm ult, it doesn't work.
Most recently, I tried to use the ult when fighting the Green Lady and the Hound, it didn't activate.
