The only one that bugs me is the one in Stonefalls that is inaccessible from that side because the "gate" is quest locked. As a long time player, I know if my character hasn't done that quest, they can get to it via The Rift and just ride through the load screen border but new players may not know that.
It's not quest locked. You run around the back of the fort around the North path. Though this does present another issue that a long time player thought it was quest locked, so newbies might too.

I was going to say this too. I groan inwardly when the Fighter's Guild daily points to certain maps since I know getting to the active dolmen is going to be a pain (and I still don't have the best routes figured out for about half of them). But other maps, you're only a few seconds away from the nearest wayshrine.
Not sure why the game is so inconsistent about this, even in the base game content.
