But then again, how can you possibly be surprised? It's simply one more thing in the long line of failures developed to turn the meta into a mess of Heavy Armor Stamina proc builds.

In first picture, you can see in Log, that you are hit with Poison enchant. It is not the Bow Light attack, proccing Viper. It is still the Poison Enchant. Thing is, it does not display on your death recap, because you die before it actually hits you. Vipers will always be the first thing hitting. So any recap like this will list;

Enchant
In the second picture it is seemingly the Lethal Arrow proccing the Viper. But my guess is, that is had Poison enchant on, which just does not show up on recap, because Mages Wrath procced on you before it was counted in. Since this ability procs based on your health percentage, it would make sense for it to cut-in between other damage types. It MIGHT also be that is procs from the Poisoned status effect, which i can see is active on you. Would make sense, since this is a DoT coming from Poison damage.
Just before ppl. start confusing things and thinking Vipers procs off anything and everything. Currently the only confirmed bug is concerning weapon enchants.

When Viper's damage was mitigated by spell resistance the bug was fixed right away. Why isnt this being addressed? or even acknowledged?
