    That's odd. Order shouldn't matter. Exactly what 3 reagents are you putting together? And exactly what is the potion effect its creating?

    Columbine+Mountain Flower+Bugloss only does health and stamina, no longer does health, stamina, magicka. I can confirm Balthyzar's issue about only getting two out of the three traits that should occur.

    I just tried out my alchemist and was able to make the (3) effect potion using: Bugloss, Columbine and Mountain flower.

    I noticed that Blessed Thistle has an attribute that ravages Health.

    Could that be the issue?

    If you're trying to make tri-stat tri-pots, you need columbine, bugloss and mountain flower. If you use Blessed Thistle, you only get stamina and magicka. You could also use columbine, bugloss and dragonthorn but the regeneration won't last as long if I remember correctly.
