Insatiable Hunger Changes clarification

Hey folks!

Confused about something in the patch notes and won't be at my PC for a bit, was wondering about this line from today's patch notes:
Put 3 unique stipulations on the synergy in addition to their regular stipulations (looking at a consumable corpse, etc)
The caster is at or below 66% Health
OR the caster is at or below 200 Ultimate
OR the caster is out of combat

Does this mean if I'm in a fight in PvE (say maelstrom arena, my chief concern here) I can't feed to bump my ulti unless one of these conditions is met?

If so...what was the impetus for this change? I might be missing it since I don't often pvp, but what was oppressive about being able to feed whenever you wanted? I'd imagine that would make you an easier target in cyro/battlegrounds/imp city, not a harder target.

Appreciate any insight!

    This isn't from today's patch notes, it's from last weeks. This week, they've removed these 3 stipulations, so the only change (so far, at least) is that the Insatiable Hunger synergy now has a lower priority
