Vestige: Noun - a trace, mark, or visible sign left by something vanished or lost
Basically, you are telling the world that you are but a shadow of your former self.

Vestige?
"That is the name I have given you. You are but a trace of your former self. A soulless one. An empty vessel that longs to be filled.
It is as the Scrolls foretold, but not exactly as I imagined."

But... in the lore AREN'T we vestiges of our former selves? Isn't that why we can use soulgems and the like?

That's the premise for the main story.

Is this screen shot from PTS? If not How did you complete the Dynamic Encounter? I did not think those were available yet.

as said, you only need to complete five. I killed a dragon, ymir world boss, the necromancer public dungeon boss in Labyrinthian, some dungeon boss in western skyrim, and a harrowing storm with my duo.

I have completed all but the Dynamic Encounter, The tile i have is just "Vestige of" there is nothing after that why does the screenshot show Vestige of your account name with the <Carried By StamDK >

The formatting is dependent on what kind of nameplate settings you're using. If you have the blue nameplates enabled above characters' heads, the title will show up either before or after the character name depending on how it's set up. (Bardok seems to have it set to show account name rather than character name.) So you might get "Lady Ilsabet Menard" or "Ilsabet Menard of House Ravenwatch" depending on how the designers want it to appear.
If you don't have the blue nameplates enabled, and just see the type with the character/account info beneath the health bar when your cursor is pointing toward the character, then the title will always show up after the account name. That's not a huge issue in most cases, but here it looks super derpy because the title only grammatically makes sense if it's followed by the character name (or account name if you ignore the @ symbol).
Also, "<Carried By StamDK>" is a guild tag.
