Challenge Incursion Breaker Achievement Bug

Khajiit has completed all Challenge Incursion Breaker achievements, but the achievement only states "Challenge Incursion Breaker 1". Khajiit is also not getting credit for Challenge Incursion Breaker 3 as part of Give No Quarter. Khajiit did all achievements on Vestige difficulty, if that makes a difference.
