It seems like corrosive + flame lash may be causing desyng or maybe it’s a bug.

You were killed with combo of Power Lash + Incinerate + Vicious Death + maybe some proc set idk, standart death recap doesn't show all info. For the Vicious Death you can thank to the ally who stood on the top left.
