Shared Burden sometimes fails to self-revive companions

I tried using Shared Burden in overworld activities to allow my companion to self-revive, because it’s hard to resurrect them during tight fights. The set works most of the time, but occasionally it fails to revive the companion. I keep the set equipped in one weapon panel and do not swap panels for over a minute while waiting for the revive, but nothing happens.

Reproduction steps (what I did):

Equip Shared Burden on one weapon panel.
Enter a combat with a companion.
Let the companion die and wait (do not swap panels).
Observe that the companion is sometimes not revived.

Expected result:
Companion should self-revive reliably when the set procs (12 sec).

Actual result:
Companion sometimes does not revive even after waiting >1 minute with the set equipped.

Additional info:

Happens in dungeons and overworld activities (not tested in trials).

Addons: Reproduced with and without active addons
OS: Windows 11 x64

Just to clarify: the active panel definitely has the Shared Burden weapon (bow), so the full 5-piece bonus is active.

Have you tried barswapping back and forth when it failed to activate?

Yes, but this doesn't work. Usually if it stops reviving the companion, /reloadui or relogging helps; otherwise it won't revive even after a wipe.

Again, the companion will be auto-revived when out of combat, but without a /reloadui or relog it won't be auto-revived in the next combat.
