I took a second character through this quest today. This time I chose the mean/violent instruments and this time, after the song was done, one of the picnic goers was lying on the ground. The one npc reaches out with her arm and the other npc against the tree collapses. Reminded me of Darth Vader's force choke, but I’m not sure if that’s what they were going for.
It happens quite quickly so you have to head to the couple by the tree as quick as possible after the “enjoy the show” line.

Well, that's something, at least. I realize ZOS isn't going to show a violent, graphic scene (particularly not during Hearts Week), but considering what the instrument vendor told us could happen, there's a bit of a disconnect there. Maybe they should just leave Sanguine out of these festivals.

February 14th is his summoning day though. Rather then leave him out, what they should have done is let the player choose how they wanted to celebrate Hearts Week. To go with the Mara way or the Sanguine way. So players who want nothing to do with the more darker holiday can be happy with the Mara stuff. While Sanguine is tempting players to come to HIS party. Also making it doable on multiple characters would give people a reason to do the quest again if they want to see both.

Wasn't there something about the instruments turning people into frenzied beasts or something like that? If they wanted to keep it less violent, the lovers could have magically turned into actual animals (predator/prey) at least, with one chasing the other, with the probably deadly outcome off-screen?
Not that it would be the best ending I could imagine, since it would still feel a bit too mild for Sanguine, but it's better than just punching each other.

This game is rated M. If Sanguine would pervert attraction in such a way that the NPCs would tear each other apart animalistically, they very well could have killed one another.

The description of attraction becoming so intense it ends with murder actually reminded me of the novel "Perfume: The Story of a Murderer" from 1985, where an insane perfumer kills people for their odour to create the "perfect perfume" that causes most intense attraction - and when he uses it on himself at the end of the story, a whole crowd of people literally becomes frenzied and tears him apart.
They also made a movie of that in 2006. Rating in my country: Ages 12+

I don't expect a gorey scene, despite the M rating noting "blood and gore." But they could have put on more of a show than what was a fast punch and fall-down given we are supposedly witnessing the consequences of an item cursed by a Prince.
I am trying not to come across as petty but if an NPC says "enjoy the show" to announce something interesting is going to happen, it should very well be noticeable and it should match the dialogue about the consequences as written.
If you're a vampire or member of the Dark Brotherhood you can get a close-up cut scene of your character violently murdering innocent people, any where and any time you can find them. I really don't think the lack of reaction to the song was because of the game's age rating or concerns about upsetting players with violence.
When I did the quest I picked the safer option but I was still surprised that I didn't see anything happen at all. Fortunately I had the sound on at the time so I did hear the music, but that was it. None of the NPCs moved at all. I was genuinely expecting the quest giver to be angry that it didn't do anything, but apparenlty it worked as intended so I assume an animation that was supposed to play wasn't working.

This is odd, because I too took more characters through it, and when I picked the other option, which might actually have been the "nicer" instrument, one npc was already lying down before I even talked to Samara.
