The Plague Doctors Axe in the main hand adds durability and survivability. While the Maelstrom Dagger in the offhand focuses more on damage output.

That’s not how set bonuses work. One maelstrom dagger doesn’t give you any bonus over a crafted non-set dagger. You need two to get the bonus from it. Same for plague doctor if you only have the axe and no other set piece, if you do, I’d suggest Trainee instead. Trainee is one of the only sets in the game that’s not a mythic and not a monster set (head/shoulder) that has a 1 piece bonus (and it’s more than a 2-piece bonus of plague doctor).

It would have been helpful to include that in your first post. SO much of the end result depends on the totality of your gear and build choices that simply identifying one or two pieces doesn't really help anyone else.
