Does this not mean that your alliance has no districts controlled in which case you wouldn't have a choice to spawn above the sewers anyway?

Right, we're saying the same thing here. If I can't spawn in the district I was in, it's better to be in the sewers near the ladders then on the surface in the wrong district.
