Losing agro with St Olms in VAS

Hi, my guild and I are working on VAS. This past Friday, I main tank and lost agro. The boss still had my taunt AND there was no other taunt on it. Just mine. It was like it told me brb and smacked the dps around. I thought ok, game glitches and we came to the conclusion it was a feature, not a bug. Several more instances have happened and OT was even seeing issues with her mini (nightblade boss). We somehow completed it. On Monday though, only 4 members were the same. Right off the bat, I taunt, he’s coming over to me and then he turned around showing me his butt and was like let me chill with the DPS.
I have went in and let another tank MT and I tried void bash, soul burst, rushing agony with crit charge and I’m not sure what else. I truly hate to post on the forums but.. is anyone else experiencing this OR have any ideas what could draw agro, not taunt, and only happen in this specific trial.

Are you sure it's an aggro problem and not that he's just turning around? He does have a problem where he can face the wrong way despite still having aggro on the main tank, unfortunately I don't remember exactly what causes it. Maybe something about main tank standing too close and bumping into his hitbox. If no one else responds with the answer, I'll go poking around for you to confirm what exactly causes him to turn.

What were you trying to accomplish with an AoE pull? There's nothing in there that responds to pulls except the little spiders.

I am the main tank on Mondays and Fridays. I was trying to replicate it again so I went back in there today with one other person. I tried going in with things that pull and don’t taunt to see if I could get him to turn around to me.
I will try to see about standing back some this Friday.
Thank you for responding. It only seems to be an issue as of lately.
