Are the ESO moon phases reversed (compared to irl), or are these tiles just mis-named?

They should be named correctly. In order from left to right in your picture:

Full moon
Waning moon
New moon
Waxing moon

Take a look at the following of the Earth's moon for comparison.

I think the confusion might lie in the order of display. If the full moon were put in the middle with the waxing on the left and the waning on the right, it would match up to the order as displayed on most images for the Earth's moon.

It's correct and matches Earth's lunar cycle. It's just that several phased are missing.

Earth vs Tamriel:

Gibbous
Third Quarter
Crescent
New
Crescent
First Quarter

I am more troubled by the fact that the moons seem to be locked in orbit with respect to each other. They would be different distances away and would not be constantly the same visual distance apart.

Simple question: which hemisphere are you from?
The sequence on the screenshot is similar to the lunar phases in Earth northern hemisphere, as the devs are from nothern one.

If you are in the southern hemisphere, then yes, it should look reversed for you. As the real life sequence for you looks like this one
