What platform are you on?
Console should still have the bug where clothing and woodworking surveys are dropping as master writs, which reduces the amount of surveys you can get by 33% (from 6 to 4 per character), or by 40% without jewelrycrafting (from 5 to 3 per character).
Any data collected on PC before September 2 would also have been impacted by this.
Before the update, the chance of getting a survey was 12.5%. Your 14-21 in 114 is a drop rate of 12.3%-18.4%, so you were getting a drop rate that varied from about what’s expected to much better than expected (almost 50% better). Your post-patch numbers are low, 6.1%-9.6%, but we don’t know if that’s due to the master writ bug, RNG giving you low drop rates after high drop rates, the chance actually changing, or a combination of some of these.
I do wonder if the drop rate for the new surveys was lowered as part of the stackable ones being unbound, but it’s going to take a lot of specific data collected after the master writ bugfix to show it.
In 1,086 writs done (that could drop surveys) on PC/NA after 11.1.6 patch (where they fixed the bugged clothing and woodworking surveys), across 3 accounts, I've gotten 132 surveys. For an average drop rate of 12.2%
181 writs done of each type.

All 6 are within 2 standard deviations of expected drop rate of 12.5%, and thus there is no statistically significant evidence that drop rates have deviated from 12.5%
If you're on Console, disregard, there is a known bug that would reduce your rate by 1/3rd since Clothing and Woodworking are bugged
