Naj-Caldeesh Trifecta Bug - we have done it, but we didn’t get the achievement (see the video)

We completed the trifecta, but we didn’t get the achievements (neither the trifecta nor No Death).
In the video, you can see that we finished the entire dungeon in 16 minutes, cleared all the trash, did the hard mode, and didn’t die a single time. But the trifecta wasn’t counted for us (in the video 17:08).

After that, we went back (in the video starting at 17:14) all the way to the very beginning to make sure we hadn’t missed a single mob.

Is this a bug? We spent a lot of time and nerves trying to get the trifecta, so please check why it wasn’t counted for us.

Ive seen several people say no death isn't working. Yet, some people also have been able to get it. Hope it gets figured out!

I wonder if its possible for an enemy to walk or be chained or knocked back into the giant hole in the ground in that one pack, because that’d cause a problem if so.

Just to say, along with the list of known-issues with achievements in Naj - the one for doing all 3 side puzzles in one run didn't proc for me and my group.

I know that is slightly off-topic, but it points to a wider issue with the Naj achievements.

I get that people THINK they might faster answer on the general discussion but I'm also pretty sure that different forum categories - housing, crafting, bugs... - exist for a reason.

Or the moderators can just move it if they deem it necessary. Seems like such a random thing to complain about.

This might not be a dungeon specific bug, various groups over the years have had bugged trifectas in dungeons and trials. ZOS policy is simply to say "unlucky, go get it again".

I can't imagine if some group progressing some hard trial like SE (Mindmender) is getting bugged trifecta after few months progress...

It did happened with Planesbreaker group. Multiple times.
