I notice this a lot on a sorc when using heavy attack lighting staff. Pressing and holding the mouse button for a HA does nothing. The sorc makes a fast "about to attack" animation, but does not carry through with the attack. I have to make multiple engagement attempts for it to fire.
I am also (in PVP) going from full health + shield to death screen without ever seeing combat. I literally just go from alive to dead as if the combat portion was edited out. Makes no sense.

Yea I thought it was my mouse and keyboard. So I went through my boxes and pulled out my old set. But it's the same feeling.
Going full alive to dead in 1 frame. Instant skills stuck at first animation frame while the world goes on around. Missing cast animations on the cast skills and just insta damage, sometimes even while I'm in cc. Tabbed to a target in front, but skills fire to the left as another player leaves or flanks the fight. Heavy and light attacks animate but no damage only the skill. Some aoe skills hitting a target Dodge rolling, being hit in the Dodge animation but not 1s after the finish of the Dodge animation.
It's a full detachment from what's going on the screen. Even when it's been horrible latency you can adjust to what's going to a degree. But there is no cadence to hold on to.

Considering you ALSO seem to have sound stuttering, are you by any chance on a wireless headset? For some very weird reason, sometimes, in such scenario, game starts lagging bad out of sudden.
