Sudden Death with No Recap?

Had this happen last night while running the Gryphon's Run WB fight with a PUG. My character suddenly died, dropping me to the re-spawn here/waypoint screen, but there was absolutely nothing in the death recap section - completely blank. I did not see any signs of an AoE or DoT, and my health bar was not showing in the screen so I didn't think I was in any imminent danger at the time.
Anyone else had this happen to them? (XB-NA)

I have had this as well PC-NA but what will they do. When it happened no recap of the event even when I hit the T on my keyboard. Some how a one shot.0
