The title says exactly what it is. since about 3 months ago until now, I noticed if I die during a boss fight in any trials, as soon as I used a soul gem to get up from my death, the game screen freeze straight away and acting like I got disconnected, but I'm VERY certain my internet is working perfectly fine when it happens. it's definitely an unsolved bug. I got disconnected 3 times in a row during a same vet Rockgrove final boss fight, all of them happened after I used soul gem to get up from my death, and My internet was working perfectly fine at the time.
