Here's what likely happened, because it's happened to me: the portal to the Index is touching the circle on the ground that begins the spawning of enemies for that stage. As you go through the portal, the stage is beginning -- but suddenly you're no longer there, and the game doesn't seem to know what to do other than counting it as a death.

Definitely something that needs fixed, but hopefully that helps explain what's going on
