✭✭✭✭✭She did that today for me as well, she poofed into a pile of ashes after yelling for help. My character had a bounty on it and I was spotted while standing around rifling in my inventory. I guess Ember took the brunt of it. I rolled away, looked back and she was ashes. Poor little thing. Then she reappeared like nothing happened. I thought companions couldn't die.
Have you ever gone Dragon hunting with your companion? They most definitely can die.
