Direfrost Keep Vet Achievements bugged (PC-NA)

A friend of mine and I just ran through Direfrost Keep. We got the achievement for Hard Mode, but not for clearing Vet. Also, according to the achievement listing, we did not kill the last boss.
We killed every single living thing in that dungeon, but apparently the game doesn't think so.
