Collectible Fragments showing Collected, won't let me combine

In my Collectibles UI under Fragments, I show that the Apple-Bobbing and Voriplasm Pet has all of the items collected, but when I try to combine them I get an error message "You do not own every collectable needed to complete this combination.

So I don't know if I am missing items what Items I am missing, or if I am missing items at all and there is something wrong with the combination UI.

i think im still missing 3 pieces of the wooden grave stake memento, but like these the collection menu does not show i have missing parts

i dont even remember the last time the fragments for this item were available, its been that long since the murkmire event lol
